Each country has its own rules about dual citizenship.  The United States allows one to be a citizen of the US and another country.  You would have to consult with a representative of the Cayman Islands (such as someone at a local consulate) about their requirements for citizenship and whether that country allows dual-citizenship.

Send your mother wedding invitation card to show to the consular officer. Also if your younger sister is not coming with your mother, make sure your mother tell the consular officer. She just has to show that her trip is temporary and she is going back to resume her life with her family in her home country.

The receipt will be sent to the petitioner, not the beneficiary.  That means your employer should get the receipt.  I do not know how long it is taking to issue receipts in the H1B program.
