What do you mean without legal ramifications? Of course, there will be legal ramifications. Plus your wife needs help and your children are in danger. I don't know if divorce is necessarily the answer; perhaps she can be treated with meds and be normal again. Regardless, you need to talk with an attorney and do what is best for your wife and your children. You may need to have her involuntarily committed for evaluation. You need to get a custody order giving you custody of the children until she is back on an even keel.

No. There is a court order stating you are to pay alimony. She can enforce that court order for alimony and a notarized promissory letter you would still have to go through the courts to try to enforce. But the promissory note itself is not enforceable on its own. You would be better off having her sign a stipulation waiving any rights to the alimony awarded by the court.
