It would not be malpractice on the part of the dr, it would be forgery perhaps, although many drs authorize their assistants and PAs and nurses to sign certain prescription refills. They often use rubber stamped signatures or metallic stamps. I suspect in this computer age they may authorize computer signatures as well.

Merely because the stairs were wet does not make the landowner responsible; you have to prove they were negligent, by either allowing a situation to exist which made the steps wet (i.e., a leaking ceiling) or they knew the steps were wet and didn't timely take corrective steps to either warn patrons of the water or clean the liquid up. If someone who was walking down (or up) the stairs just a few minutes ahead of you spilled their drink, that landowner wouldn't be responsible under that factual scenario, but the burden is on you to prove how the water had gotten there or how long it had been there.

While it is true that your exclusive remedy against your employer  for an "on the job injury" is workers comp, you may be able to sue the owner of the dog on a strict liability theory. Of course, this depends on facts unique to each case. One factor is whether you contractually waived any claim for damages against a third party dog owner when you were hired. Inspection of your work related documents that you signed may be relevant to that analysis.
