If you are undocumented and entered without a visa or inspection, you may not be eligible to file for adjustment of status (I-485). I would highly recommend consulting with an immigration attorney before filing. If you were inspected and overstayed your visa, you might be eligible for adjustment but I would still consult an immigration lawyer. You don't want to loose those steep I-485 filing fees.
If you are undocumented and entered without a visa or inspection, you may not be eligible to file for adjustment of status (I-485). I am assuming that your visa says PED April 2015. Depending on what occurred next, you may or may not be out of status. You should schedule a consultation before you leave with an attorney and have them review your documents to make sure that you can get back into the US.

If you self-sponsored yourself under EB-1 as an extraordinary alien, changing jobs would have no effect on your green card.
