It depends upon the documentation of your relationship. There are two to three parts to the process at both USCIS and the State Department. I strongly recommend an appointment with a competent and experienced immigration attorney, where you want the process to go smoother and thereby faster, but not fast. It can take about a year, perhaps longer. Some may be disqualified.

No, if your spouse has not entered into the USA, then the I-485 is not needed since the I-485 is an application to adjust one's status from one current status to another based upon a petition.

Although you did not mention the type of visa for which you applied, generally the USCIS can be expected to issue an Employment Authorization Document (an "EAD" or "work permit") within 75 - 90 days of filing of a Form I-765 application associated with an application for adjustment of status.
