If you had your own B2 visa in your passport, that would be proof now that your legal status expired.  The application for a replacement I-94 card is typically used by those whose status has not expired and they want to be able to prove they are here legally.  If you came on a B2 visa in 1990, your status would have expired six months after you entered in that status (unless you extended, in which case it might have been 1 year after you entered).  Why do you believe you were in status between 1991 and 2012?  In any case, you should be able to prove your status has expired using your visa.  The airline ticket will help prove that you used your visa in 1991.
