You've just discovered the perils of trying to serve as your own attorney. There is no court approved form for what you want to do. Your best option is to contact a local attorney and get the proper papers drafted. It shouldn't be that expensive.

How do you get a divorce from your spouse if you dont know where he resides and you have not had contact for 12 years?

In New York State, you are required to have your spouse personally served with divorce papers before a divorce can be granted.  If you personally have had no contact with your spouse, nor know anyone who may know where he may be located, then the next step would be to hire a private investigator to locate him.  If the private investigator is unable to locate him after an extensive search, then they would prepare an "Affidavit of Due Dilligence."  With this, you could make an application for alternate service, such as "service by publication" if you can at least locate the town or city that he last resided in.
