Name, address and amount owed are the most important. Creditors get your SSN and compare it to ALL accounts in their databases. Account numbers are not used by the Court nor by most creditors.

You cannot incur any new debt while in a 13 without court approval. Your "borrowing" money against your house would require approval. If borrowing these funds and the resulting monthly payment affects your distribution to creditors under the plan then you will not be given permission to enter into the loan.
