I wish I knew what a PPI is. If you mean permanent impairment rating, the psychologist can assign a rating using the AMA guidelines. It may be that a psychologist is not qualified and that you would need a psychiatrist. If your doctor refuses to issue an impairment rating, you may need to pay for an independent evaluation. If this is not what you are asking, then I need some clarification as to what PPI is.

The gym may have its own insurance for medical bills incurred due to injuries, under what might be called Medical Payments coverage. That should be checked. Also you need to find out if a Release of Liability or Waiver was signed. If so, you need to see if this complies with the law. If it does, then you will likely not be able to sue for the injuries. If not, or if there was not a waiver, then you might want to look at a potential lawsuit for negligent supervision. This would allow you to recover more then just the medical bills.
