So for example, if the medical bills they paid total $3,000 and your attorney charged you 1/3 in fees and costs, the medical bills get reduced by 1/3 (i.e., $1,000) so you only pay them back $2,000. However, depending on the circumstances the amount reimbursed can be negotiable and sometimes even waived; a lot depends on how much you recovered in relation to the amount the insurer paid.

You need to ask your boss for some training. Your legal terms are not even correct. You don't waive a pre-formal hearing, you are requesting one if it is traffic.
