I am unaware of any medical cause for drug testing outside of a mandate in an incarceration setting unless consensual or medically necessary where consent cannot be given. However, were I a physician with cause to believe that a patient might have pain medication in his/her system which if present would materially assist me in addressing addiction or toxicity issues and the patient denied me testing I'd immediately reject the patient; such patients are not worth the malpractice liability risks they pose.
