Will I have to serve jail time for my first DUI?

Very doubtful. Most likely, you'll be able to plea it down to impair and get a one year probation and restrict driver's license.

If I am driving on a suspended license under a car registered to a different person, can the police know I'm the driver?

If you get stopped, your ID will be verified through the computer system and you would be arrested for driving on suspended. If you try to give false ID to police, which is a separate criminal charge. the picture associated with that drivers license number would show it isn't you.

What am I looking at on a second DWI?

If your first DUI was in 2002 and your second was in 2013, you will be punished as though this was your first. Your punishment will be enhanced because of the refusal.
