It really depends upon the facts surrounding the dismissal. You are not eligible to re-file a bankruptcy case for 180 days if (1) the case was dismissed because you failed to abide by Bankruptcy Court Orders, or (2) you voluntarily dismissed the case following the filing of a motion for relief from the automatic stay. In most other circumstances you could re-file immediately, although you will have to jump through a few hoops to get the automatic stay extended beyond the 30 day period after the re-filing. This may or may not be a problem, depending upon the type of bankruptcy that you file and what you are hoping to achieve by such a filing.

You are allowed to change the type of case you have. You would not file a new Chapter 13 case if you're already in an active Chapter 7 case, but you can file the appropriate pleadings to have your existing case "Converted." There are a few criteria, so occasionally the debtor's request for Conversion may draw opposition and perhaps even be blocked or denied. But Converting cases between Chapter 7 and Chapter 13 is something permitted under bankruptcy law.

It all depends on the type of creditor. If it is an unsecured creditor you would need to amend Schedule F to list the creditor. If it is a secured debt then you would need to add them to Schedule D and amend that. If they are a priority creditor you would need to add them to Schedule E. Once you add them and amend the schedule you should also file an amended creditor matrix so that when the case is discharged the court will mail them a copy of the discharge order.
