You might want to keep the medical authorization part, it will allow the physicians to give medical treatment without waiting for parental consent. Although, in an emergency they would legally have to treat anyway, even without written consent. The other part purporting to give a release is probably overreaching, and invalid. Just to be safe you probably should cross it out.
