I was injured while on light duty; I never was reported as injured worker. Employer and insurance carrier denied or limit workers comp benefits without justification, & forced me to work in violation with No Work Order from doctor. Injuries both shoulder surgeries, and lumbar lami L5-S1 with fusion. . He misleads me to sign fraudulent documents and incorrectly filed my medical records, & filed a fraudulent Petition for Benefits with date of accident that never happens.
