The first question is whether you are a U.S. citizen. If yes, you are eligible to petition on behalf of a spouse or fiance (not a girlfriend) assuming she has no impediment barring entry to the U.S.

First, if you are a US citizen, the gap in your wife's status will not affect her AOS process. As to the medical exam: recently the rules regarding its submission have changed. Please check USCIS website for more information.

Assuming you're a USA citizen or green card holder, waiting 6 years is way too long. You're obviously doing something wrong. You should hire a board certified immigration lawyer to fix the problem and do the case properly.Feel free to contact me if you want a lawyer to handle it.
