First you need to be divorced. I find it is faster if the couple is married rather than fiance but each case is different. I recommend working with an attorney, my firm handles such cases in NY.

The imposition of the bar to re-entry has to be reversed here, in the U.S. Application for a new visa will only waste your fiance's time and money, since the embassy has no power to reverse the bar (and, if they ignore it, your fiance has a nearly 100% chance of being turned around at the port of entry).

Sometimes they suspect fraud or desire to further investigate. Sometimes the security checks have not cleared. There are potentially dozens of possible reasons.
