Well, you may have to pay that debt first, before you can apply for a new visa. You see, the US is not interested in paying your medical bills and allowing you to come in and incur yet more bills.

Hello. You have several options. First, as a married US citizen, your children born outside of the US should be US citizens.  You need to go to the nearest US embassy to file the proper paperwork. As for your husband, you can certainly start his paper work while living in El Salvador but when he has his appointment at the US embassy you'll need to have US residence so that he can permanently reside in the US until he become a naturalized US citizen. Please call me for further advice pertaining to yoru particular case.
