Probate is very likely going to be necessary. Given the complete lack of estate planning, the only way you wind up with the house is if you are the only heir, or if all other heirs agree to let you have it. You will almost certainly need an attorney.

The nursing home does not take cars. The question is whether it the car is an exempt asset for purposes of Medicaid. I suggest that you speak with an elder law attorney in the jurisdiction where your mother lives to address all of her assets and getting her situated for applying for Medicaid.
