Yes - a trust can be formed.  You can allow the trust to accept the prize and do not have to have your name announced.  People often do that because if you win the lottery, you will be inundated with requests for money.  Keep in mind you will have a time deadline to accept your lottery prize.

There is no concrete answer to this question. To adequately address your options you should speak with an attorney and address the timing of the POD account addition, the medical condition of the decedent and your options. We charge $100 for a one hour consultation with an attorney who will provide you with important information regarding your specific case and will able to advise you on the options that you should consider in determining your next steps, or if it involves reviewing documents and preparation in advance of the meeting, we charge $300 for the consultation, because there is over one hour of review and preparation for the meeting and one hour for the meeting.

It is not possible to answer this based on the information given.  If you are trying to avoid estate taxes, the money will be drawn back into the estate, if the person dies within 3 years o the gift.  In order to avoid the taxes, the person must give up all indicia of ownership of the asset.  I prefer to let elderly people keep their assets to pay for their own care instead of worrying about taxes.  estate taxes will nt be inucrred unless the estate is over $675,000
