An unexecuted Quadro is of absolutely no legal effect. If your divorce judgment or property settlement called for the Quadro to be executed and your ex refused to do so, then you should have your lawyer follow up with him. You can take him back to court, if need be. The court might even sanction him for failing to comply.

Can we have their names removed off the property deed now that they are deceased?

You may need to go through probate in order to take the two deceased siblings off the deed. Their portion of the property would be a part of their estate if the property was titled in their names and there was no TOD (transfer on death designation)associated with their interest.

You need to provide more information. I know of no "estate affidavit" that would be issued by the probate court. There is an "Affidavit of Decedent's Successor" form, which is NOT a probate proceeding, which should do the trick for you, provided there is no probate estate and the value of the check in question is less than $22k.
