You should have received an itemized bill showing an accounting for the expenditure of the retainer. If you did not receive it or if you were not billed properly you have certain rights - among them the right to arbitrate at minimal cost. You should use one name for all purposes. As you have seen, using different names gets confusing and some government agencies won't accept it. So, decide what name you want to use, and systematically change it with every agency that has you under the different name. It doesn't matter whether you're married, separated or divorced.
