If you married your second wife without divorcing the first one, the second marriage is invalid. When you will petition the USCIS for an immigrant visa for your second wife, you will have to show that your first marriage was legally dissolved before you entered into the second. The only solution to your problem is 1) obtaining a divorce from your first wife in the U.S.; 2) obtaining an annulment of your second marriage in the country where you married her; and 3) getting married to your second wife, again. Only after you clear your second marriage you can apply for a permission for your second wife's entry and residence in the U.S.

Your divorce should have addressed this. Yes, if the house was given to you in the divorce you can refi (you should have been ordered to do so) and she/he will need to sign a quit claim deed as part of the process (this too should have been in the property settlement/decree.

If you are separated from your husband while an adjustment case is pending, and reconciliation is not something you both foresee, it would be advisable to withdraw the application in lieu of facing a denial. Your employment authorization is linked to your pending adjustment of status application, therefore, you will be unauthorized to work when application is withdrawn. It would be good to invest in formal consultation with an immigration lawyer.
