There is no "form" for this sort of thing. You need either a stipulation or a motion, depending upon whether your ex agrees with the proposed change - and you need a properly drafted QDRO.

Your daughter is absolutely entiled to spousal maintenance (alimony) as part of the divorce- even if she were NOT handicapped. Given that she is handicapped, she might be entitled to more than the statutory amount.
