Exactly which way your documents have to be worded will vary a little bit depending on who your judge is. Each district does it slightly different.

You may be able to convert the case or follow it with a Chapter 13. You should speak with your current attorney.

Can I file for bankruptcy again?

It depends. If you filed a Chapter 7 before, you can file again 8 years from the filing date of the prior Chapter 7. You can file a Chapter 13 if your prior Chapter 7 was filed more than 2 years ago. Congress tacked on another year between 7 filing.
