With that income it is very unlikely that you would qualify for a chapter 7, but it depends on how many kids you have and what other expenses you have.

Can only one spouse file if both share the income? How?

The answer to that question hinges on whether your wife is personally obligated on any of the debt, i.e. did she sign any note, is she a responsible party on a credit card, is any of the medical debt due to care for her for which she might be responsible to pay. If any of these responses are affirmative, you should probably both file. If, however, she has never signed an application for credit or been a signer on a note, or an accommodation party, that may not be necessary. You should gather all your documentation and sit down with a competent bankruptcy attorney to determine the best process for your situation.
