The fact that the hospital bill is not shown on your credit report could have several explanations: it's too old, the hospital doesn't report to the credit reporting agencies any more, someone lost the file, etc. To be on the safe side, you should list the hospital in the bankruptcy, as well as all other real and potential or doubtful claims against you. You need to talk to an experienced bankruptcy attorney, the sooner the better. Most will give you an initial consultation for a small fee, or none at all.

You can no longer file bankruptcy in the Eastern District of Michigan without paying all the attorney fees before filing. You can apply to pay the $306 filing fee in installments. There was a very recent bankruptcy case decision that decided attorneys cannot collect fees from clients after the bankruptcy has been filed. This has the effect of shutting down the bankruptcy "mills" that enabled debtors to start for as little as $100.00 and pay the remaining fees and costs after the case was filed. I am not certain how this decision in the Eastern District is being treated in the Western District.

Well, you can file your own bankruptcy case and not tell your husband about it. If he is not co-liable on any of your debts he does not have to be listed as a creditor in the case and will not get notice from the Court. However, if you and your husband are commingling your income and expenses, then you have to list his income and expenses on your bankruptcy schedules and you will have to list all of the income he has received in the last 6 months in the means test form. So, you will need to be able to get this information somehow.
Well, you can file your own bankruptcy case and not tell your husband about it.
