What do I do if I have not heard from the lender for six years but I do need the title?

If you wanted title to the car you had to pay for it. The debt has already been discharged, but the title will not be in your name. When you filed you had to indicate that you intended to keep the collateral or surrender it.

First of all, you cannot discharge IRS debts. Secondly, you are now single and no longer married to him, so you will still get your refund.
